Crazy Karting Bolton - Customer Waiver

Assumption of Risk, Waiver, and Release of Liability

By completing the form above I hereby acknowledge and agree to the following terms and conditions before participating in any activities at Crazy Karting Bolton:

Assumption of Risk: I understand that go-karting involves inherent risks, including but not limited to, the risk of injury or death resulting from collisions, loss of control, mechanical failure, and other unforeseen circumstances.
Waiver of Liability: In consideration for being permitted to participate in go-karting activities at Crazy Karting Bolton, I hereby waive, release, and discharge Crazy Karting Bolton, its owners, employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of any damage, loss, or injury, including death, that may be sustained by me or my property while participating in such activities, whether caused by the negligence of Crazy Karting Bolton or otherwise.
Indemnification: I agree to indemnify and hold harmless Crazy Karting Bolton, its owners, employees, agents, and affiliates from any loss, liability, damage, or cost, including attorney fees, they may incur due to my participation in go-karting activities at Crazy Karting Bolton.
Safety Responsibility: I acknowledge that it is my sole responsibility to ensure my own safety during the go-karting experience. I agree to abide by all safety rules and instructions provided by Crazy Karting Bolton staff and to use the provided safety equipment properly.
Medical Clearance: I certify that I am physically and mentally fit to participate in go-karting activities and that I have no medical conditions that would prevent me from safely participating.
Consent: I consent to receive medical treatment that may be deemed necessary in the event of any injury, accident, or illness that occurs during my participation in go-karting activities at Crazy Karting Bolton.
Photography and Video Release: I grant Crazy Karting Bolton the irrevocable right and permission to photograph and/or videotape me during my participation in go-karting activities and to use such photographs and/or videos for promotional purposes without compensation to me.
Legal Capacity: I affirm that I am of legal age and competent to enter into this waiver, or that I am the legal guardian of the participant and have full legal authority to sign this waiver on their behalf.

I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
